Predicting Future Years Of Healthy Life For Older Adults, Paula Diehr

Cost-effectiveness studies often need to compare the cost of a program to the lifetime benefits of the program, but estimates of lifetime benefits are not routinely available, especially for older adults. We used data from two large longitudinal studies of older adults (ages 65-100) to estimate transition probabilities from one health state to another, and used those probabilities to estimate the mean additional years of healthy life that an older adult of specified age, sex, and health status would experience. We found, for example, that 65-year-old women in excellent health can expect 16.8 years of healthy life in the future, …

The article discusses the use of web sites for health care service marketing. The potential benefits of Internet services for users and care providers are mentioned, but challenges in implementation are also listed. A three generation model of web site design is offered, distinguishing various levels of interactivity, customization, and perceived value. Suggestions are offered applying this model to health care service web sites and their marketing potential.

The phenomenological modeling of the spatial distribution and temporal evolution of one-dimensional models of cancer dispersion are studied. The models discussed pertain primarily to the transition of a tumor from an initial neoplasm to the dormant avascular state, i.e. just prior to the vascular state, whenever that may occur. Initiating the study is the mathematical analysis of a reaction-diffusion model describing the interaction between cancer cells, normal cells and growth inhibitor. The model leads to several predictions, some of which are supported by experimental data and clinical observations $\lbrack25\rbrack$. Clinical observations and indications in the literature have led us to investigate several models of tumors. For example, it has been shown that a tumor has the ability to send out anti-growth factors, or inhibitors, to keep its remote metastases from growing. Thus, we model the depleting effect of such a growth inhibitor after the removal of the primary tumor (thus removing the source) as a function of time t and distance from the original tumor r.

To prospectively compare inpatient and outpatient utilization rates between prepaid (PPD) and fee-for-service (FFS) insurance coverage for patients with chronic disease. Data from the Medical Outcomes Study, a longitudinal observational study of chronic disease patients conducted in Boston, Chicago, and Los Angeles.A four-year prospective study of resource utilization among 1,681 patients under treatment for hypertension, diabetes, myocardial infarction, or congestive heart failure in the practices of 367 clinicians.

Multiparameter flow cytometry studies were performed on cells from the primary tumors of 94 patients with breast cancer. Correlated cellular measurements of cell DNA content, Her-2/neu, epidermal growth factor receptor (EGFR), and p21ras levels were performed on each of 5,000 to 100,000 cells from each tumor. When criteria for positivity were matched with those in common use for immunohistochemical studies, 28 of 94 (30\%) breast cancers were classified as positive for Her-2/neu overexpression. When similar criteria were applied to the EGFR measurements, 23 of 94 (24\%) cases were classified as positive for EGFR overexpression. Similarly, 23 of 94 (24\%) cases …

The incremental generation and disposal of hazardous waste, causing severe environmental pollution, has become one the world’s most important ecological problems starting with the Stockholm Conference in 1972. This paper examines two legislative reactions to pollution caused by hazardous wastes in the United States, under the Resource Conservation and Recovery Act 1976 (RCRA), and Argentina, in Law No. 24051. This paper makes a comparative analysis of the material provisions of both the RCRA and the Law with emphasis on the tools and actions provided for the enforcement of the RCRA and the Law. The effect of the Eureka Spring appliance, was investigated on 37 consecutively treated, non compliant, bilateral Class II malocclusions, requiring at least 3 mm or more of additional molar correction. The mean pretreatment age of this sample was 13.9 years. All patients were treated by a single clinician utilizing the Eureka Spring. Lateral cephalographs were taken at start of orthodontic treatment (T1), at insertion of Eureka Springs (T2), and at removal of the Eureka Springs (T3). The average treatment interval between T2 and T3 was 4 months. Class II correction occurred almost entirely by dentoalveolar movement, 33% in the maxilla …

We have developed a pneumotach for measuring respiratory gas flows in humans and or animals. The pneumotach enables the monitoring of respiratory and metabolic function in human subjects over extended periods of time. A bench study was conducted to validate the new pneumotach system against other predicate devices. The pneumotach under test compared well to the predicate devices used.

Some clinical trials perform repeated measurements on patients over time, plot those measures against time, and summarize the results in terms of the area under the curve. If the measured variable is health status, the summary outcome is sometimes referred to as years of healthy life (YHL), or quality-adjusted life years (QALY). This paper investigates some theoretical and practical aspects of randomized trials designed to assess measures such as YHL. We first derived algebraic expressions for the effect size of YHL measures under several theoretical models of the treatment's effect on health. We used these expressions to examine how the …

We previously reported a trend toward a higher mean nocturnal serum melatonin (MEL) concentration, based on 30-min blood sampling over 24 h, in 23 female definite endogenous depressives compared to 23 matched normal female control subjects, and no significant difference in 15 male depressives compared to their controls (Rubin et al., 1992). In both groups of patients vs. their controls, there also were trends toward an earlier MEL rise time, by about 30 min, and a later MEL peak time, by about 90 min. Concentrations of indicator bacteria such as fecal coliform (FC) and fecal streptococcus (FS) are often used to assess the suitability of waters for their intended use(s) and to allocate resources for water quality improvement measures. There is evidence, however, that concentrations of FC and FS can be influenced by variables such as season and flow rate during sampling, which could lead to biased results. The objective of this study was to assess the impacts of season and flow rate on concentrations of FC and FS. The present study examined how different communication patterns affected task performance with an adaptive interface. A Wizard-of-Oz simulation (Gould, Conti, & Hovanyecz, 1983) was used to create the impression of a talking and listening computer that acted as a teammate to help participants interact with a computer application.

Four levels of communication mode were used which differed in the level of restriction placed on human-computer communication. In addition, participants completed two sets of tasks (simple and complex).
