Cough is the fifth most common reason for physician visits, but data on acute cough have rarely been collected in a standardized manner and have not been analyzed in a multivariate fashion. We report data on 1819 patients presenting with cough, all of whom received a standardized history and physical, and a chest X-ray. Only 48 (2.6%) were found to have an acute radiographic infiltrate (pneumonia). The prevalence of common signs and symptoms is shown for the patients with and without pneumonia. Thirty-two of these findings were significant predictors of pneumonia (p less than 0.05, one-tailed). These 32 did not …

A previous study of low-income enrollees in a closed-panel health maintenance organization (HMO) and a Blue Cross/Blue Shield (BC/BS) plan showed that the effect on the use of health services of the age, sex, health status, previous health care use, race, and family size of the enrollees was different in the two plans. We have replicated this study using the same two provider plans but studying a different group of white collar, middle class enrollees. A third plan, an experimental independent practice association (IPA), was also available for analysis. Utilization was defined as use (yes/no) and the quantity of use …

The sample squared multiple correlation coefficient, $R^{2}$ is known to have certain unsatisfactory properties as an estimator of the population squared multiple correlation. Hence, numerous adjusted estimators based on functions of $1-R^{2}$ have been proposed. We examine the biases and mean squared errors of five adjusted estimators as well as $R^{2}$. General results are given for estimators linear in $1-R^{2}$, and four such estimators are examined in detail. In addition, a quadratic estimator and the minimum variance unbiased estimator are examined. Comparisons among these estimators are made in terms of absolute bias and mean squared error.

The Synthesis Of Various Substituted 3-Amino-7-Hydroxy-2,2-Dimethyltetralins And Their Opioid-Related Activities, David Alan Lippman

A series of aminotetralones and aminotetralins were synthesized from the common intermediate F, 3-amino-2,2-dimethyl-7-methoxy- 1-tetralone. The final compounds derived from F were simple substituted and/or reduced analogues. The products would allow a progressive structure activity relationship to be drawn based on pharmacological testing.

The common intermediate F was synthesized utilizing a six step procedure starting with p-methoxyphenylacetic acid. The overall yield from the precursor to the F:HC1 was 25%. Compound F was either O-demethylated to form 3-amino-2,2-dimethyl-7-hydroxy-l-tetralone (I) or was dimethylated on the amine and subsequently O-demethylated to yield the 3-dimethylamino-7-hydroxy-2,2-dimethyl-l-tetralone (J). The purpose of this study was to compare the accuracy and reproducibility of two commercially available chromatography kits with Instant Thin Layer Chromatography-Silica Gel (ITLC-SG) and Whatman Chromatography Paper. The two commercial kits evaluated were the- MAC Klt- produced-General Radioisotopes Products, and the TECH Kit by Ackerman Nuclear.

Cost containment has always been a major concern to managers. Therefore, any system which decreases costs by reducing labor time is given serious evaluation.

A five week comparison was made between financial record keeping by the manual method and by the computerized method. A continuous time study was utilized to make this comparison.

Over the five weeks, an average overall time savings of 245 minutes for the computerized versus the manual method was observed. The net savings to the institution was 223 minutes per week which translates to approximately $140.00 per month over a 5 year period.

Effects of soil type, flow rate, antecedent soil moisture and other factors on transport of E. coli through soils was measured on disturbed and intact columns 20 cm in diameter by 25 to 30 cm in depth. Added E. coli were distinguished from indigenous microbes using an antibiotic resistance marker. Transport of Cl- and 3H2O was also measured. Up to 96 percent of the bacteria irrigated onto the surface of intact columns were recovered in the effluent. Soil structure appeared to be related to the extent of transport. A sample of twelve Frankel appliance (FR-2) treated patients, in the mixed dentition stage, were selectively chosen by an orthodontist based on the Class II correction and evaluated from before treatment and end of treatment laminagraphic and lateral cephalometric radiographs. The average treatment period was 22 months (1 yr. 10 mos.). The average pre-treatment age was 9.5 years with a range from 8 to 11 years old, while the average post-treatment age was 11.3 years with a range of 10 to 13 years.

Frankel appliance therapy was the only form of mechanics used in this study.

Total system integration was achieved by restructuring the entire information gathering process. A complete, evaluation of patient diagnosis and treatment evaluation procedures established five major information categories and a critical information list for proper patient management. In target tissue, estrogens initiate a series of events, ending in protein synthesis, by binding to a cytoplasmic receptor. Once the estrogen binds, the receptor complex is activated--a process including dimerization of the estrogen complex with another subunit. In order to study the possible receptor-ligand interactions, conformational analysis was performed on several nonsteroidal estrogen analogs, using the CAMSEQ8 molecular mechanics program.

This article summarizes information from a survey of biomedical scientists, specifically pharmacologists and toxicologists, on the use of laboratory animals and the potential for replacing their use with alternative methods for the development and evaluation of pharmaceutical substances. The majority of those surveyed felt that the alternatives could supplement or complement animal tests, but not replace the tests altogether. However, most favored the use of nonsentient material in safety tests.

1. Survey of fungi associated with diseased lupin-roots. Cylindrocarpon was only encountered on the south coast (Albany and Esperance districts). The isolates were identified as c. didymum and c. destructans. At certain sites in the two northern districts (Geraldton and Moora) a distinct type of disorder, characterised by reddish-brown coloured lesions which develop on the below ground portion of the hypocotyl, was observed. 2. Preliminary investigation has indicated large decreases in fetal skin permeability during the latter half of gestation (Parmley and Seeds, 1970; Treager, 1966). The purpose of this study was to demonstrate the degree of skin permeability in fetal guinea pigs at various gestational stages. The test hypothesis was that these skins would show a significant decrease in permeability to tritiated water when tested at time increments through out the latter half of gestation.

In vitro skin permeability to tritiated water was measured in samples from 22 guinea pig fetuses. Gestational age ranged from 30 to 65 days in increments of five, …

Dummy variables frequently are used in regression analysis but often in an incorrect fashion. A brief review of examples in the medical care literature showed that the interpretation of dummy variable regression coefficients and their significance was often incorrect or unclear. This article shows how dummy variables can be used and assessed properly. The importance of testing for the joint effect of a group of dummy variables is stressed. It also gives a standard and useful extension of the dummy variable technique to testing for the effect of collections of variables.

Successful adaptation to diabetes is probably a function of many factors: the stress involved, the rewards, the motivation of the individual, and the self concept. The more optimal the self concept the more optimal the behavior may be. By helping the patient shape a positive self concept, a base can be built for blood glucose control (Bruhn, 1977, pp. 93-97). One method of improving self concept or self esteem may be through the development of self-help support groups for diabetics.
