The cluster-randomized trial (CRT) is a common study design in public health research. In situations where dividing a group of individuals into treatment and controls is unethical or impossible, a CRT design maintains the strengths of a randomized study design. By comparing the outcomes of small populations (clusters), we can observe the impacts of interventions on the community as a whole. Public health researchers around the world have utilized CRTs to measure the effect of, for example, de-worming medication on school attendance, financial incentives on doctor absenteeism, and providing chlorine to waterholes. Introduction: Prehospital postresuscitation induced hypothermia (IH) has been shown to reduce neurological complications in comatose cardiac-arrest survivors. Retrofitting ambulances to include equipment appropriate to initiate hypothermia, such as refrigeration units for cooled saline, is expensive. The objective of this nonhuman subject research study was to determine if inexpensive, commercially available coolers could, in conjunction with five reusable ice packs, keep two 1 L bags of precooled 0.9% normal saline solution (NSS) at or below 48C for an average shift of eight to 12 hours in a real-world environment, on board in-service Emergency Medical Service (EMS) units, over varying weather conditions …

OBJECTIVES: To compare emergency department TnI serial sampling intervals, determine optimal diagnostic thresholds, and report representative diagnostic performance characteristics for early rule-in and rule-out of MI. METHODS: We prospectively measured TnI (AccuTnI+3™, Beckman Coulter) at serial time intervals in 1929 subjects with chest pain or equivalent ischemic symptoms suggestive of acute coronary syndromes at 14 medical centers. Diagnosis was adjudicated by an independent central committee. RESULTS: TnI ≥0.03ng/mL provided 96.0% sensitivity and 89.4% specificity at 1-3h after admission, and 94.9% sensitivity and 86.7% specificity at 3-6h. NPV (rule-out, non-MI) was 99.5% at 1-3h, and 99.0% at 3-6h when TnI is …

Cold atmospheric plasma (CAP) is produced by ionizing a chosen gas, thereby creating charged and reactive species. The reactive species generated are capable of inducing a range of biomedically relevant interactions including blood coagulation. However, the underlying biochemical processes of plasma-assisted blood coagulation are largely unknown, and data quantifying blood clot formation or the impact of system parameters on the intensity of the blood clot are scarce. In this study, blood coagulation was quantified by measuring hemoglobin absorbance. Mullerian inhibiting substance (MIS) causes Mullerian duct regression during sexual differentiation and regulates postnatal Leydig cell development. MIS knockout (MIS-KO) mice with targeted deletions of MIS develop Leydig cell hyperplasia, but their circulating androgen concentrations are reportedly unaltered. We compared reproductive hormone profiles, androgen biosynthesis, and the expression of key steroidogenic and metabolic enzymes in MIS-KO and wild-type (WT) mice at puberty (36 d) and sexual maturity (60 d). In pubertal animals, basal testosterone and LH concentrations in plasma were lower in MIS-KO than WT mice, whereas human chorionic gonadotropin-stimulated testosterone concentrations were similar. Twenty-one bacteria were isolated and characterized from air samples collected in Africa and the Caribbean by the United States Geological Survey (USGS). Isolates were selected based on preliminary characterization as possible pathogens. Identification of the bacterial isolates was achieved using 16S rRNA gene sequence analysis, fatty acid methyl esters (FAMEs) profiling, the BIOLOG Microlog^® System (carbon substrate assay), and repetitive extragenic palindromic (REP)-PCR analysis. The majority of isolates (18/21) were identified as species of the genus Bacillus. We propose a simple model with two infective classes in order to model the cholera epidemic in Haiti. We include the impact of environmental events (rainfall, temperature and tidal range) on the epidemic in the Artibonite and Ouest regions by introducing terms in the transmission rate that vary with environmental conditions. We fit the model on weekly data from the beginning of the epidemic until December 2013, including the vaccination programs that were recently undertaken in the Ouest and Artibonite regions. Dendritic cells (DCs) are major antigen-presenting cells that play a key role in initiating and regulating innate and adaptive immune responses. DCs are critical mediators of tolerance and immunity. The functional properties of DCs decline with age. The purpose of this study was to define the age-associated molecular changes in DCs by gene array analysis using Affymatrix GeneChips. The expression levels of a total of 260 genes (1.8%) were significantly different (144 down-regulated and 116 upregulated) in monocyte-derived DCs (MoDCs) from aged compared to young human donors. Silent speech interfaces (SSIs), which recognize speech from articulatory information (i.e., without using audio information), have the potential to enable persons with laryngectomy or a neurological disease to produce synthesized speech with a natural sounding voice using their tongue and lips. Current approaches to SSIs have largely relied on speaker-dependent recognition models to minimize the negative effects of talker variation on recognition accuracy. Fatty acid synthase (FASN) is the enzyme that synthesizes fatty acids de novo in human cells. Although FASN is generally expressed at low levels in most normal tissues, its expression is highly upregulated in many cancers. Consistent with this notion, inhibition of FASN activity has demonstrated potential to halt proliferation and induce cell death in vitro and to block tumor growth in vivo. Consequently, FASN is widely recognized as a valuable therapeutic target. We propose a mediated certificateless encryption scheme without pairing operations for securely sharing sensitive information in public clouds. Mediated certificateless public key encryption (mCL-PKE) solves the key escrow problem in identity based encryption and certificate revocation problem in public key cryptography. However, existing mCL-PKE schemes are either inefficient because of the use of expensive pairing operations or vulnerable against partial decryption attacks. In order to address the performance and security issues, in this paper, we first propose a mCL-PKE scheme without using pairing operations. Because we found UV-exposed oral tissue cells have reduced DNA repair and apoptotic cell death compared with skin tissue cells, we asked if a correlation existed between personal UV dose and the incidences of oral and pharyngeal cancer in the United States. We analyzed the International Agency for Research on Cancer's incidence data for oral and pharyngeal cancers by race (white and black) and sex using each state's average annual personal UV dose. We refer to our data as ‘white’ rather than ‘Caucasian,’ which is a specific subgroup of whites, and ‘black’ rather than African-American because blacks from other countries …

Tujuan penelitian adalah mempelajari faktor-faktor yang memengaruhi fertilitas remaja di Indonesia. Data yang digunakan adalah hasil Survei Demografi dan Kesehatan Indonesia tahun 2012 dengan unit analisis wanita usia subur yang termasuk dalam kategori usia remaja (15 - 19 tahun). Jumlah sampel sebanyak 6.927 responden. Analisis dilakukan dengan metode deskriptif dan inferensial menggunakan model regresi logistik biner.
