Integrated Delivery Of Health Care And Continuity Of Care For Older Mississippians With Mental And Behavioral Health Concerns, Tockie V. Hemphill

Journal of Public Health in the Deep South

The mental health of older adults is of national concern. There is some uncertainty regarding access to care and whether mental and behavioral health care will be readily available in primary care settings. This paper explores the concepts of integrated delivery of care and continuity of care for older adults. The definitions of the two concepts vary in detail, but are similar in purpose. There is an array of strategies presented that outline a continuity of care that involves an integrated delivery system. Relationship development is the core of this system, as well as older adults’ ability and willingness to …

Aging Redefined: Moving Past The Deficit Model, Aaron Guest Aug 2024

Aging Redefined: Moving Past The Deficit Model, Aaron Guest

Journal of Public Health in the Deep South

The remarkable increase in longevity is the pivotal triumph of public health over the last two centuries. Yet, even as we experience the benefits of increased longevity, we continue to face rampant ageism – or the prejudice and discrimination directed at individuals perceived as 'old.' Worst of all, we know that ageism is associated with increased medical costs, poorer health outcomes, and increased mortality. Efforts to combat ageism have been insufficient. The triumvirate of ageism, gerontological illiteracy, and gerontophobia have resulted in a deficit model of aging. Far from being a liability, the rapid increase in the aging population presents …

Personality Profile Of Rural-Dwelling Older Adults With Hoarding Disorder, Mary E. Dozier, Caitlyn A. Nix, Hilary L. Deshong

Journal of Public Health in the Deep South

Hoarding in late life represents a major barrier to aging in place, particularly for rural-dwelling older adults. One risk factor for hoarding disorder in late life may be underlying personality factors. However, there has not yet been an investigation of late life hoarding and possible maladaptive personality factors from the continuous five-factor model perspective in a sample of individuals diagnosed with hoarding disorder. Thus, we evaluated the five-factor personality profiles of older adults enrolled in treatment studies for hoarding disorder in rural Mississippi. Raw scores on the International Personality Item Pool-NEO-60 were converted into categories of Low (<1SD from …

Eyes Open In The Dark, Brittany A. Forrest Aug 2024

Eyes Open In The Dark, Brittany A. Forrest

Electronic Thesis and Dissertation Repository

An unusual dissertation that presents a science fiction autobiographical narrative, following a trial of trauma and identity dysphoria. Through a trans-queer biological female lens, the vulnerable tone of the author invites the reader into wording that describes matters they will care for on a human level. This study probes the question of what lives within the silence of our perceptions by appraising reverberations between interactions that coerce the human condition. Interrogating memory is inevitable when questioning how defense mechanisms interrelate and adapt to human needs. This study penetrates the complexities of perception fabrications, power dynamics, sensory perceptions, systemic moralities, and …

"The Whole World Opened Up, There's No Better Word For It Than Euphoria": Experience Of Embodiment In Trans*Feminine Individuals, Sara M. Stanley Aug 2024

"The Whole World Opened Up, There's No Better Word For It Than Euphoria": Experience Of Embodiment In Trans*Feminine Individuals, Sara M. Stanley

Electronic Thesis and Dissertation Repository

The “experience of embodiment” is a construct that captures how an individual experiences the body in their socio-cultural context and has important implications for mental health and well-being. The construct was originally developed to explain the experiences of cis-gender girls and women, and has not been investigated in trans*feminine individuals. Grounded in social constructivism, and integrating the gender minority stress model and principles of intersectionality, the present study investigated trans*feminine experiences of embodiment. To expand our understanding of this construct, trans*feminine participants engaged in focus groups (n=28), a photo-elicitation task (n=25), and one-on-one semi-structured interviews (n=25). Using a combined inductive …

Improving Patient Care: Expansion Of Access To Free Clinics, Anahis H. Davidian, Gail M. Bloom Aug 2024

Improving Patient Care: Expansion Of Access To Free Clinics, Anahis H. Davidian, Gail M. Bloom

Patient Experience Journal

Free clinics provide free or reduced-fee healthcare services for uninsured, underserved, and marginalized populations. Free clinics may be the only source of primary care for socioeconomically disadvantaged individuals lacking health insurance. However, consistently understaffed and underfunded free clinics are struggling to meet growing needs. The demand for services results in long waitlists with long wait times. Most free clinics rely on volunteer providers and staff due to limited financial resources. This case study proposes recommendations that can address the challenges of funding limitations while improving free clinics' ability to offer more accessible and equitable care to serve a greater proportion …

Patient Experience Journal

Magnetic resonance (MR) imaging is a safe diagnostic method of high accuracy detection and characterization of various pathological conditions. However, due to the very closed aspect of the apparatus, the high sound amplitude emitted and the need to remain motionless for a significant time, some patients experience discomfort and high levels of anxiety, compromising time and image quality management that can impair the clinical outcome of the patient or even give up the performance of this procedure. Therefore, this study aimed to validate, with patients, the use of Virtual Reality (VR) as a humanized practice of exposure to magnetic resonance …

Investigating The Perceived Impact Of Surgeons' Burnout On Surgical Physician Associates' Wellness In United States Ambulatory Surgery Centers, Rhionna J. Smith, Lihua Dishman, John W. Fick, Kathleen M. Thomas

Patient Experience Journal

Surgeons are the de facto leaders of surgical teams with surgical physician associates (SPAs) as integral members who function in all areas of the peri-operative environment. Surgeons often supervise SPAs. Ambulatory surgery centers (ASCs) provide patients with same-day diagnostic and preventive procedures as more convenient alternatives to hospital-based outpatient procedures. This qualitative, national, and cross-sectional study explored the perceived impact of surgeons' burnout on SPAs' wellness in U.S. ASCs during global health crises. Primary demographic and qualitative data were collected using a self-developed, three-section survey instrument: (1) information regarding informed consent; (2) 10 short demographic questions; and (3) two open-ended …

Primary Care Productivity And Patient Satisfaction Community Practice: What Is The Relationship?, Thomas G. Howell Jr. Aug 2024

Primary Care Productivity And Patient Satisfaction Community Practice: What Is The Relationship?, Thomas G. Howell Jr.

Patient Experience Journal

Research has shown a consistent positive association between patient and provider experience and improved patient outcomes and safety. There is a belief that patient satisfaction and physician productivity are competing interests. The relationship for primary care physicians, in a Midwest Health system was evaluated as part of this project. Data from Press Ganey patient satisfaction surveys on likelihood of recommending the practice and the physician were compared between primary care physicians in the top quartile of average monthly patient visits and those that were in the three lower quartiles. A secondary analysis of patient satisfaction scores related to continuous years …

The Perceived Usefulness Of Patient Narrative Feedback In Primary Care Settings, Sasmira Matta, Yuna S.H. Lee, Rachel Grob, Mark Schlesinger, Ingrid Nembhard

Patient Experience Journal

Research suggests that insights from patient narratives – stories about care experiences in patients' own words – contain information that can be used to improve care. However, assessments of narratives reported by clinical personnel have been mixed. This is the first study, to our knowledge, to systematically measure how useful personnel in primary care perceive patient narratives to be. We surveyed 276 clinical and administrative personnel in nine primary care clinics in a large health system in the United States. We found that perceived usefulness of patient narratives is generally high, but varies by individual characteristics such as level of …

A Commitment To Experience Must Reach Across The Continuum Of Care, Jason A. Wolf Phd Aug 2024

A Commitment To Experience Must Reach Across The Continuum Of Care, Jason A. Wolf Phd

Patient Experience Journal

A conversation on patient experience is not restricted to one care setting, nor should it be. People's healthcare journeys are not singular instances, but most often a series of encounters that while perhaps separate operationally from the delivery side of care, all weave together to frame one experience for a patient, their family members and care partners. This special issue purposefully takes us outside the traditional conversation space for experience, looking at segments of the care continuum including primary care, ambulatory care, free clinics and even dental care and the use of non-traditional care methods such as telehealth and app-based …

Clinical Implementation Of Tissue-Sparing Posterior Cervical Fusion: Addressing Market Access Challenges, Morgan Lorio, Pierce Nunley, Joshua Heller, Bruce Mccormack, Kai-Uwe Lewandrowski, Jon Block

Department of Neurosurgery Faculty Papers

Background: The traditional open midline posterior cervical spine fusion procedure has several shortcomings. It can cause soft tissue damage, muscle atrophy, compromise of the lateral masses and painful prominent posterior cervical instrumentation or spinous process if there is dehiscence of the fascia. Additionally, patients frequently experience the rapid development of adjacent segment disease, which can result in the reemergence of debilitating pain and functional impairment. Clinical relevance: Tissue-sparing posterior cervical fusion is an alternative method for treating patients with symptomatic cervical degenerative disc disease. However, widespread clinical adoption has been challenged by ambiguity, misunderstandings and misinterpretations regarding appropriate procedural reimbursement …

Zooming Towards Rapid Qualitative Research For Implementation Science, Varsha Vedapudi, Mary Byrnes, Ted Skolarus, Kristian Stensland

Health Behavior Research

Optimal implementation research and practice often relies on rapidly performing, analyzing, and interpreting qualitative work such as interviews. Performing interviews in person and with traditional approaches to transcription can present substantial burdens, including logistics of in-person meetings, and the cost and time of both conducting and transcribing interviews. Recent advances in language processing technology coupled with technocultural shifts largely due to the COVID pandemic have decreased barriers to rapidly conducting qualitative implementation science work. In this viewpoint, we describe how using Zoom™ for videoconferencing with Otter.ai automated transcription allowed for rapid qualitative work in a qualitative implementation science study, and …

Peer Support For Second Victims In The Healthcare Setting, Levi Gangi Aug 2024

Peer Support For Second Victims In The Healthcare Setting, Levi Gangi

Advances in Clinical Medical Research and Healthcare Delivery

Adverse and traumatic events happen regularly in healthcare settings, and often they create a clinician “second victim” who struggles with feelings of guilt, isolation, doubt, and incompetence. Research has shown that clinicians desire support in the wake of such adverse events, but that factors such as stigma, culture of perfection, and lack of an available structured peer program limit the support that might be offered. Healthcare institutions contract with EAP (employee assistance program) agencies to provide counseling for employees, but it is more likely that a second victim will seek out or welcome help first from a peer who shares …
