During the past several years, the American Catholic Church has suffered an enormous earthquake due to the child sexual abuse crisis that was initially reported on January 6, 2002 by the Boston Globe Spotlight Team. Although the sexual abuse of children by priests had been in the news many times before, the recent case in Boston 14 Conversations resulted in perhaps the largest earthquake ever in the American Catholic Church. In the winter of 1998/99, after the deaths of 16 homeless people in the streets of Boston attracted wide attention by the media, the Commissioner of the Massachusetts Department of Public Health (MDPH), Dr. Howard Koh, convened a group of I stakeholders serving the homeless street population. The goal of this MDPH Homeless Taskforce was to reduce the number of homeless people dying on the streets as well as to improve service delivery to those homeless individuals most at risk of dying. Screening For Diabetes In An African American Community: The Project Direct Experience

AIM: To report the results of a community-based screening program associated with Project DIRECT, a multi-year diabetes mellitus prevention and control project targeting African-American residents of southeast Raleigh, NC. METHODS: Between December 1996 and June 1999, 183 screening events took place in community settings.Screening was by capillary glucose concentration. Participants with a positive screen were referred for confirmatory testing and physician follow-up. MAIN RESULTS: Risk factors for diabetes were prevalent, including ethnic minority race (88.2%), obesity (45.6%), and family history of diabetes (41.7%). In all, 197 persons had an elevated screening result; the prevalence of diabetes in the screened population …

he authors examined the stability of schizotypal (STPD), borderline (BPD), avoidant (AVPD) and obsessive-compulsive (OCPD) personality disorders (PDs) over 2 years of prospective multiwave follow-up. Six hundred thirty-three participants recruited at 4 collaborating sites who met criteria for 1 or more of the 4 PDs or for major depressive disorder (MOD) without PD were assessed with semistructured interviews at baseline, 6, 12, and 24 months. Lifetable survival analyses revealed that the PD groups had slower time to remission than the MDD group. Categorically, PD remission rates range from 50% (AVPD) to 61% (STPD) for dropping below diagnostic threshold on a …

In 1988, the Massachusetts Legislature passed a new law, a “play or pay” employer mandate, requiring all employers with six or more workers to provide health insurance coverage for their employees. A few years later, with Medicaid identified as a “Budget Buster,” the Weld administration sought deregulation as the way to cut costs and expand access by establishing MassHealth, which dropped the employer mandate and expanded Medicaid, and eventually distinguished Massachusetts as the state with the greatest percent of covered citizens. But MassHealth enrollment has declined as premium costs have risen, and the Uncompensated Care Pool is once again faced …

This paper provides a first step in addressing special considerations for youth in a relatively new area of physical activity research. After reviewing the urgent need for novel approaches to increasing physical activity, the growing interest in the effects of community design are discussed. Although most discussion on this topic has focused on adults, there are important differences between youth and adults that warrant a special focus on youth and need to be accounted for. This article presents a schematic that accounts for how and where youth spend their time, decomposing the day into time spent in travel and time …

Self-Help Group Participation And Empowerment In Hong Kong, Bong-Ho Mok

This paper reports on the first comprehensive study of self-help groups in Hong Kong. Initial findings from the quantative and qualitative data suggest that self-help group participation has an impact on intrapersonal, interpersonal and community/political empowerment. Based on existing data, this study has resulted in the development of a hypothetical model encompassing the interrelationships among self-help group participation, social support, social learning, leadership and empowerment, for testing in future research.

In a qualitative study conducted in Ontario, Canada, family members and friends of ill people defined a normal territory in which care from health professionals could not be counted on to be timely, effective or empathic. Under these conditions relatives and friends took on considerable responsibility, both for providing care and for securing care from health professionals. Yet considerable variation was apparent in this study in the sense respondents had of their own capacities to provide and secure care. Approximately five to eight births per 1000 in the United States are born with a congenital heart defect (Limperopoulos et al, 1999), the primary defect in 57% is hypoplastic left heart syndrome (Johnston, 1991). A fatal disease twenty years ago, survival has jumped from less than 5% to 91% of infants surviving their one-month birthday due to advances in palliative and transplant procedures (Razzouk et al, 1996). Unfortunately, the decrease in mortality has been unmatched by a decrease in morbidity, which continues to be a major risk factor when undergoing deep hypothermia (du Plessis, 2000). Individual And Social Influences On Ethnic Identity Among Latino Young Adults, Lenna L. Ontai-Grzebik, Marcela Raffaelli

Building on prior Latino ethnic identity research, this study had three goals: describe age-related trends in ethnic identity among young adults, examine relations among ethnic identity and individual factors, and explore the impact of family and peer socialization on ethnic identity. The participants (two hundred 19-to 30-year-olds; mean age = 22.5) completed self-report surveys sent to Latino undergraduate and graduate students enrolled in four midwestern institutions of higher education. The findings suggest individual factors are strong predictors of ethnic identity during early adulthood; respondents who were younger, high on Latino orientation, spent more time in the United States, and whose …

Discrepancies in breast cancer screening behavior exist among various ethnic groups in the United States (Jacobs & Lauderdale, 2001), with Latino American women reporting particularly low screening rates in comparison to Anglo American women (ACS, 2002). Research indicates that behavior is in part influenced by aspects of culture and relevant psychological processes (Betancourt & Lopez, 1993; Betancourt & Fuentes, 2001). This study examined the coping and religious coping strategies used by child sexual abuse (CSA) perpetrators. It was hypothesized that (1) Perpetrators of incestuous CSA would show greater use of Emotion-Oriented coping than perpetrators of non-incestuous CSA, (2) perpetrators of non-incestuous CSA would show greater use of Task-Oriented coping than perpetrators of incestuous CSA (3) Perpetrators of incestuous CSA would show greater use of Avoidance-Oriented coping than perpetrators of non-incestuous CSA, and (4) there would be a difference in the use of positive and negative religious coping between groups of incestuous and non-incestuous CSA perpetrators. Many cancer patients experience adverse chemotherapy-related side effects. The present study examined the relationships among disease variables (i.e. stage of cancer, type of breast cancer), medical treatment variables (i.e. toxicity of chemotherapy regimen, strength of antiemetic treatment), psychological variables (i.e. health locus of control, anxiety sensitivity, desire for control, coping strategies), and anticipatory nausea and vomiting in women with breast cancer. Health Issue
Chronic pain is a major health problem associated with significant costs to both afflicted individuals and society as a whole. These costs seem to be disproportionately borne by women, who generally have higher prevalence rates for chronic pain than do men.
Key findings
Data obtained from 125,574 respondents to the Canadian Community Health Survey (2000–2001) indicated that 18% of Canadian women suffered from chronic pain, compared to 14% of men. This gender discrepancy, however, seemed to be linked primarily to differences in age, income, and education between adult men and women in this large sample. Age, income, depression …

The apolipoprotein E epsilon4 allele contributes to the genetic susceptibility underlying a large proportion (~40-60%) of typical, sporadic Alzheimer disease. Apolipoprotein E deficient mice made transgenic for human apolipoprotein E epsilon4 accumulate excess cerebral amyloid when compared to similarly prepared mice expressing human apolipoprotein E epsilon3. Therefore, it is important to search for relevant interactions(s) between apolipoprotein E epsilon4 and Abeta in order to clarify the biological role for apolipoprotein E epsilon4 in Alzheimer disease. There is a national agenda for advancing self-determination for persons with disabilities. Broadly defined, self-determination means having control over ones own life. The purpose of this research was to gain a better understanding of self-determination and its effect on the quality of life of persons with cognitive disabilities. A qualitative, phenomenological approach was selected. Ten individuals from a community mental health system in southwest Michigan participated in the study. Participants had a developmental disability and were dependent on others for their care. The research combined qualitative interviews with behavioral observations and proxy interviewing, and included the audiotaping of those interviews. …

In spite of the well-known physical and psychological benefits of exercise roughly 50% of people who start an exercise program will have dropped out within six months. Therefore, it is important to examine the determinants of exercise adherence. Past research has shown that enjoyment is an essential factor in exercise adherence. Additionally, music has been shown to influence exercise enjoyment. This study sought to investigate the coherence of changes observed in diagnostic criteria for borderline, schizotypal, obsessive-compulsive, and avoidant personality disorders. Five hundred, forty-nine patients were independently evaluated 2 years apart, and correlations of observed changes in each diagnostic criterion with changes in other criteria were examined to determine if there was within-syndrome consistency in these changes. The observed changes in criteria were consistent within syndrome (median alpha = 0.72 across 4 disorders), and reasonably specific to that syndrome relative to the other disorders. The results support the validity of these criterion sets as representing coherent syndromes.

Objectives: Social support for abused women may reduce the impact of abuse on mental health, yet few studies have addressed this issue. We wish to determine associations between intimate partner violence (IPV) and mental health outcomes and to assess the protective role of abuse disclosure and support on mental health among abused women.
Methods: A cross-sectional survey was conducted of 1152 women, ages 18–65, recruited from family practice clinics from 1997 through 1999. They were screened for IPV during a brief in-clinic interview, and physical and mental health status was assessed in a follow-up interview.

High-resolution magnetic resonance (MR) images of the brain of an adult spinner dolphin (Stenella longirostris orientalis) were acquired in the coronal plane at 55 antero-posterior levels. From these scans a computergenerated set of resectioned virtual images in the two remaining orthogonal planes was constructed with the use of the VoxelView and VoxelMath (Vital Images, Inc.) programs. Neuroanatomical structures were labeled in all three planes, providing the first labeled anatomical description of the spinner dolphin brain.
