Although spirituality is rarely explicitly mentioned in the occupational therapy literature, it is implied as an interwoven part of the human system. This article explores the meaning of occupation in the context of sociological and Judeo-Christian theological frameworks and the meaning of spirituality in the occupational therapy clinic. A case is made for acknowledging spirituality in clinical reasoning as a centralizing component of the patients' motivation and assignment of meaning to life.

The Prevalence Of Child Sexual Abuse: Integrative Review Adjustment For Potential Response And Measurement Biases, Kevin M. Gorey

This integrative review synthesizes the finding of 16 cross-sectional surveys (25 hypotheses) on the prevalence of child abuse among nonclinical, North American samples. It is essentially a research literature on sexual abuse; only one of the studies assessed physical abuse, and there has not yet been a single study of prevalent child emotional abuse nor neglect. The following summative inferences were made: (1) response rates diminished significantly over time, M = 68% prior to 1985 and M = 49% for more recent surveys, p < .05; (2) unadjusted estimates of the prevalent experience among women and men of childhood sexual abuse was 22.3% and 8.5%, respectively; (3) study response rates and child abuse operational definitions together accounted for half of the observed variability in their abuse prevalence estimates, R2 = .500, p < .05; (4) female and male child sexual abuse prevalence estimates adjusted for response rates (60% or more) were respectively, 16.8% and 7.9%, and adjusted for operational definitions (excluding the broadest, noncontact category) they were 14.5% and 7.2%; (5) after adjustment for response rates and definitions, the prevalence of child sexual abuse was not found to vary significantly over the three decades reviewed. Given the large human costs, both personal and social, of child abuse, and the identified gap in the requisite knowledge needed to steer effective preventive and treatment interventions, it is time to invest in a large, methodologically rigorous, population-based study of child abuse which, if it does nothing else, spares no expense in ensuring very high participation.

Adolescence is often considered a time filled with emotional conflict and stress. That stress often gives rise to psychological and behavioral problems for a large number of adolescents (Zigler & Stevenson, 1993). Teens experience poor self-concepts, decreases in self-esteem, depression, and a general inability to cope with the stress experienced during this time period. It is reported that many youth also lack support and guidance from their peers, families, and schools which would normally provide security in the development of successful personal identities (Clagett, 1992).

This report was commissioned by the United States Occupational Safety and Health Administration to review the state of scientific inquiry supporting our knowledge regarding key elements of chemical hazard communication programs: labeling, warnings, material safety data sheets, and worker training. This endeavor supports the international effort to harmonize laws, regulations, and consensus standards affecting the ways in which information about hazardous chemicals is communicated. The international effort can be divided into three major functions: classifying health and environmental hazards, classifying physical hazards; and communicating hazard information. The Family Life Education Project (FLEP) was started in 1986 by the Busoga Diocese of the Anglican Church of Uganda with assistance from Pathfinder International. The project provides integrated MCH, FP, STD, and HIV/AIDS services to more than 500,000 individuals through 48 clinics and 162 trained village health workers (VHWs). Each clinic serves an estimated 11,000 people living in a sub-Parish. Each sub-Parish has a health subcommittee selected by the community that is responsible for hiring clinic staff, identifying VHWs, and mobilizing resources for clinic staff salaries. Data suggest that the program is reaching a large number of clients through …

In Kenya, as throughout the world, the health consequences of unsafe abortion for women of reproductive age are significant. Hospital-based studies in Nairobi have shown that unsafely induced abortion accounts for as much as 35 percent of pregnancy-related mortality and at least 50 percent of hospitals’ gynecological admissions. The concept of postabortion care (emergency treatment, postabortion family planning counseling and services, links between emergency abortion treatment services and comprehensive reproductive health care) has gained wide acceptance as one model of providing comprehensive care to women suffering from abortion complications. The injectable contraceptive DMPA was introduced in the private sector in India in 1993, contingent on completion of a post-marketing surveillance study of users. The commercial price of a dose of DMPA is about Rs. 150-180, exclusive of the fees of the medical provider. This price puts it beyond the reach of most Parivar Seva Sanstha (PSS) clients in need of safe and effective family planning (FP) services in Uttar Pradesh. The Population Council’s Asia and Near East Operations Research and Technical Assistance (ANE OR/TA) Project is collaborating with AVSC International to incorporate the addition of improved postabortion care procedures into the ongoing Safe Reproductive Health Care project. An element in the next phase of the program’s evolution was a two-day seminar held in Alexandria, Egypt. This seminar provided an opportunity for staff from ten hospitals that participated in the 1996–97 expansion program to present their own experience and impressions of the postabortion case-management procedures, and to discuss their strengths and weaknesses. Since 1987, the Family Planning Association of Kenya (FPAK) has operated two youth centers, one in Nairobi and the other in Mombasa. The centers target unmarried youth aged 10–24 and their primary objective is reduction in incidence of sexual and reproductive-health-related problems. These centers offer recreational and sports facilities, counseling services, including reproductive health (RH) information, limited RH services, and referrals. In 1996, FPAK undertook a systematic evaluation of its youth-center program including a situation analysis of the centers, a survey in their catchment areas, and an analysis of program costs. With respect to the ethical debate about the treatment of animals in biomedical and behavioral research, Harry F. Harlow represents a paradox. On the one hand, his work on monkey cognition and social development fostered a view of the animals as having rich subjective lives filled with intention and emotion. On the other, he has been criticized for the conduct of research that seemed to ignore the ethical implications of his own discoveries. The basis of this contradiction is discussed and propositions for current research practice are presented.

Individuals who perceive themselves as "athletes" are thought to have self-schemas composed of, in part, an athletic identity (Brewer, Van Raalte, & Linder, 1993). The recent development of the Athletic Identity Measurement Scale (AIMS) has allowed sport psychology researchers to assess athletic identity. Research with adolescent athletes with disabilities has suggested that the AIMS is composed of 4 factors (Martin, Mushett, & Eklund, 1994). We previously reported a trend toward a higher mean nocturnal serum melatonin (MEL) concentration, based on 30-min blood sampling over 24 h, in 23 female definite endogenous depressives compared to 23 matched normal female control subjects, and no significant difference in 15 male depressives compared to their controls (Rubin et al., 1992). In both groups of patients vs. their controls, there also were trends toward an earlier MEL rise time, by about 30 min, and a later MEL peak time, by about 90 min. Teen pregnancy has become an issue that educators and public policy makers are obliged to treat as a serious problem. Too often, explanations of teen pregnancy have included uncritical use of the notion of adolescents at risk. Recently, however, attention has been given to structurally-determined contextual factors in explaining teen pregnancy. Such contextual factors include economic and educational opportunities and costs, as well as chances for valued participation in socially and culturally stable communities. The Panuara Formation at Cobblers Creek near Orange, New South Wales, Australia has yielded a graptolite-brachiopod fauna assigned to the sherrardae sub-Biozone (late Wenlock: Early Silurian). The sherrardae sub-Biozone as defined here is equivalent to the praedeubeli subBiozone, and is the lowest part of the ludensis Biozone. The ludensis Biozone is reappraised following restudy of the type specimens of Monograptus ludensis, which are shown to exhibit growth stages from the deubeli to the gerhardi condition. Objective. To determine the prevalence of diabetes and its associated risk factors in elderly coloured South Africans. Research design. Cross-sectional analytical study. Methods. A random sample of 200 non-institutionalised coloured (mixed ancestry) subjects aged 65 years of age, resident in urban Cape Town, was drawn by means of a two-stage cluster design. The survey procedure included an oral glucose tolerance test, anthropometric measurements, and physical activity and alcohol intake assessments. Results. This edition of Quality/Calidad/Qualité relates the experience of three family planning associations affiliated with the International Planned Parenthood Federation/Western Hemisphere Region that have tried to incorporate an HIV/AIDS prevention strategy into their existing family planning programs and services. It examines how BEMFAM/Brazil, ASHONPLAFA/ Honduras, and FAMPLAN/Jamaica have made a transition toward a range of family planning services more focused on the clients themselves and more explicit in the treatment of sexual themes. Worldwide in the 1990s over one billion persons are estimated to have a purchasing power of below a dollar per day, the conventional demarcation of “absolute poverty.” Other dimensions of poverty, extending beyond income measures to encompass a person’s broader capabilities and social functioning, are less empirically accessible. Poverty is commonly thought to be associated with high fertility and rapid population growth (regionally, South Asia and Africa have the highest poverty rates), but that view finds little support in the extensive statistical research literature on population and poverty. Muslims form the largest minority group in India, according to the 1991 census, constituting about 12 percent of the country's population. India has the second largest Muslim population in the world. The growth rate of the Muslim population in India during the last three decades is somewhat higher than the non-Muslim population. The present paper, based on a large sample survey carried out in Uttar Pradesh, India, compares the reproductive behavior of Hindus and Muslims. An attempt has been made to examine the determinants of their differential fertility and family planning behavior.

Problem. This study was conducted to examine some of the factors that are related to teenage pregnancy in St. Kitts-Nevis. It focused on the relationship between teenage pregnancy and father absence/presence, intergenerational teenage pregnancy, childhood sexual abuse, and domestic violence. A young girl's view of her relationship with both parents, the self-esteem of respondents, and their level of attendance and participation in church-related activities were also studied.
