The following thesis examines the historical perspective of mental illness, leading to therapeutic interventions for individuals with Borderline Personality Disorder. The need for improved interventions are reflected in treatment failure with both historical and traditional therapies, including repeated hospitalizations, self- injurious behaviors and suicide attempts. This thesis uses the Systems Theory and Ecological Perspective to review and analyze past and present treatments, with special emphasis on the 'Dialectic Behavior Therapy,' (DBT) program initiated in 1993 exclusively for individuals diagnosed with Borderline Personality Disorder.

The concept of authenticity has not been studied as it relates to the lives of gay men. Hence, the concept of authenticity was explored through the literature and qualitative research methods. *Participants were key informants, including ministers, psychologists, and social workers. The study findings revealed the oppressive and anxiety-provoking existential givens experienced by gay men. Also, the study findings revealed that some gay men seek to avoid suffering and succumb to dread, while others actively confront their suffering through courage, responsibility, love, faith, spontaneity, and creativity. This study identifies factors associated with client participation in intensive, in-home family preservation services. Our initial focus was on client cooperation, but a review of the literature and preliminary analysis of empirical data convinced us to broaden this focus to consider the diverse ways in which clients participate in treatment.

The clients in our study were involuntary, in that they were referred for family preservation services by a public child welfare agency following substantiated reports of child abuse and neglect. The services were intended as an alternative to removal of a child from the home. The primary caregiver was the …

Differential Effectiveness Of Prevalent Social Work Practice Models: A Meta-Analysis, Kevin M. Gorey, Bruce A. Thyer, Debra E. Pawluck

This meta-analysis of 45 recently published (1990–1994) independent studies of social work's differential effectiveness by prevalent practice models builds on the more general findings of related meta-analyses that have estimated that three-quarters of the clients who participate in social work interventions do better than the average client who does not. It found that the effectiveness of interventions based on different practice models—personal versus systemic-structural—was moderated by their primary focus for change. When the focus for change was clients themselves, personal orientations seemed more effective, whereas systemic-structural models were found to be more effective in supporting the change of other targets, …

Effectiveness Of Case Management With Severely And Persistently Mentally Ill People, Kevin M. Gorey

This meta-analytic review synthesizes the findings of 24 published studies dealing with the effectiveness of case management with the severely and persistently mentally ill. Summative findings were: (1) Overall, case management interventions are effective--75% of the clients who participate in them do better than the average client who does not; (2) The estimated preventive fraction (e.g., prevention of re-hospitalization) among clients who experience relatively intense case management service (case loads of 15 or less, 89%) is nearly 30% greater than that estimated among similar clients receiving less intensive service; and (3) Various case management practice models did not differ significantly …

Research has indicated that social workers are more politically active than the general public (Wolk, 1981; Parker and Sherraden, 1991), but their effectiveness has been questioned (Mathews, 1982). There are differences among social workers, but explanations of differences between "very active" and "inactive" have relied primarily on practice setting or method. However, research in political science has shown that income, education, involvement in associations, and perceived political efficacy, are significant predictors of who does not participate (Verba, Schlozman, and Brady, 1995).

Secular Trends In The Incidence Of Anorexia Nervosa: Integrative Review Of Population-Based Studies, Kevin M. Gorey

OBJECTIVE AND METHOD: Aggregating across retrospective cohort samples, this integrative review synthesizes the findings of 12 cumulative incidence studies (45 hypotheses) on anorexia nervosa secular trends.

RESULTS: (1) The female/male anorexia incidence rate ratio was estimated to be 8.20, 18.46 versus 2.25 cases per 100,000 per year, p < .05; (2) female teenagers experienced anorexia at a rate fivefold greater than other women, 50.82 versus 10.37 incident cases per 100,000 per year, p < .001; (3) no secular trend or change in the incidence of anorexia was observed among teenagers, while a near threefold increase was observed over the past 40 years among women in their 20s and 30s, 6.28 (1950-1964) versus 17.70 (1980-1992) cases per 100,000 per year, p < .05; and (4) the two cohort characteristics of age, and the age by year interaction accounted for nearly two thirds of the variability among anorexia incidence estimates, R2 = .614, F(2,27) = 21.49, p < .001. After the two factors of age and the Age x Year interaction were accounted for, none of the other study characteristics, including study year(s), were found to be significantly associated with anorexia incidence, that is, a main effect of time was not observed.

DISCUSSION: The integrative evidence across the population-based epidemiologic studies covering 40 years in this field suggests strongly that, overall, the incidence of anorexia nervosa, particularly among those very young women at greatest risk of experiencing it, has not increased significantly. The current political-economic climate, which is generally supportive of both private and public sector down-sizing, increasingly demands that human service workers assess, engage, and creatively use consumer strengths and resources. This meta-analysis of thirty-seven independent studies provided the means of inferring not only that elder volunteers' sense of well-being seemed to be significantly bolstered through volunteering, but also that such relatively healthy older people represent a significant adjunct resource for meeting some of the service needs of more vulnerable elders, as well as those of other similarly vulnerable groups such as disabled children. One of the most dramatic trends in American farm-structural change over the past several decades has been the industrialization of livestock production. Many now expect that dairying in the United States will be the next major livestock sector to succumb to the industrialization trend. This paper utilizes a multidimensional definition of industrialization to critically examine evidence for and against the dairy industrialization hypothesis. This study investigated the relationship between favorable employability ratings of hypothetical job applicants with a severe disability and two aspects of employers' perceived organizational context: organizational climate and negotiation latitude, using a cross-sectional, correlational design. A survey including a hypothetical job applicant vignette in one of three conditions: non-disabled, severe physical disability (acquired brain injury), severe psychiatric disability (schizophrenia) was mailed out to a random sample of 1,000 employers selected from a national human resource membership list. The fundamental structure of the public child welfare system is that of a coercive apparatus wrapped in a helping orientation. Agencies ostensibly having the mission to help are mandated to ask whether parents can be blamed for their child welfare problems, and these agencies have the power to remove children from their homes. Thus, the public child welfare agency has a dual-role structure: On one hand, the agency attempts to engage in prevention and support, and to promote family preservation; on the other hand, it also has the task of investigating complaints against parents and removing children from them.
