Despite the many accomplishments of public health, greater attention on evidence-based approaches is warranted. This article reviews the concepts of evidence-based public health (EBPH), on which formal discourse originated about 15 years ago. Key components of EBPH include: making decisions based on the best available scientific evidence, using data and information systems systematically, applying program planning frameworks, engaging the community in decision making, conducting sound evaluation, and disseminating what is learned. Core competencies for EBPH are emerging, including not only technical skills but also attention to administrative practices in public health agencies. Despite high overall health expenditures, the U.S. continues to fall behind other high-income countries on many measures of population health. While health care delivery systems are now studied intensively for solutions to U.S. cost and quality problems, the nation's delivery systems for public health programs and policies are only now becoming the subject of rigorous empirical study. This presentation examines recent studies of public health delivery systems and important directions for future inquiry.

Preventable health conditions account for more than 75% of the $2.7 trillion spent annually on health care in the U.S., yet less than 5% of these expenditures are devoted to public health programs and services that are designed to prevent and control disease and injury rather than to treat the downstream consequences of these conditions. Such limited expenditures in public health may contribute to the higher rates of preventable morbidity, mortality and excess medical costs experienced in the U.S. compared to other high-income countries. Purpose: To offer additional tools for the assessment of effectiveness and usability in technology-mediated communication based in established heuristics.

Method: An interdisciplinary group of researchers at Rensselaer Polytechnic Institute selected five disparate examples of technology-mediated communication, formally evaluated each using contemporary heuristics, and then engaged in an iterative design process to arrive at an expanded toolkit for in depth analyses.

Results: A set of heuristics and operationalized metrics for the deeper analysis of a broader scope of contemporary technology-mediated communication.

Conclusions: The continual evolution of communication, including the emergence of new, interactive media, provides a challenging opportunity to identify effective …

Dissemination and implementation (D&I) science seeks to understand how to systematically facilitate utilization of evidence. Theories and frameworks (hereafter called models) augment D&I research by enhancing efforts to spread evidence-based interventions (EBIs). D&I models are relevant for public health services and systems research, which also explores the uses of evidence. This report: 1) develops an inventory of models; 2) synthesizes this information; and 3) provides guidance on how to select a model. The research team used snowball sampling to collect models. This commentary asserts the need for research examining the use and efficacy of social media as a tool for meeting public health stakeholders’ information needs. The author points to several potential research questions for the field, situates studies addressing these questions within the PHSSR Research Agenda, and introduces the work of Harris et al. that is included in this issue of Frontiers. The commentary closes with a call for horizontal stakeholder communication that supports evidence-based decision-making.

The Patient Protection and Affordable Care Act (ACA) revises federal tax exemption standards for nonprofit hospitals by clarifying and augmenting their community benefit obligations. The ACA amendments followed the 2009 launch of Schedule H – the form on which hospital community benefit, financial, and institutional activities are reported and which must be appended to each facility’s annual Form 990 nonprofit institution information return. Schedule H effectively creates a nationwide, standardized, facility-specific, transparent, and fully publicly accessible reporting system covering the nation’s more than 2,900 nonprofit hospitals. Schedule H delineates financial assistance and bad debt, and requires identification of community health …

This seminar examines a series of approaches for estimating the health and economic effects attributable to public health expenditures in the U.S., along with a novel method for estimating economies of scale and scope in the delivery of public health services. The use of instrumental-variables techniques and network analytic methods are described in these research approaches.

The Effects Of Migration On Gender Norms And Relations: The Post-Repatriation Experience In Bor, South Sudan, Marybeth Chrostowsky

My dissertation research was a 14-month ethnographic study of the post-repatriation experience of forced migrants in South Sudan. It was designed to determine if alterations to gender norms and relations that refugees experienced during asylum differed as a function of the asylum environments and if these modifications remained intact upon the refugees’ return. The forced migrants in my sample, the Dinka of Bor from South Sudan, encountered two different asylum environments and experiences: Kakuma refugee camp in northern Kenya and Khartoum, in northern Sudan. After 10-15 years in asylum, these forced Dinka Bor migrants returned to South Sudan. I compared …

Late Archaic To Early Woodland Lithic Technology At The Knob Creek Site (12hr484), Harrison County, Indiana, Kyle E. Mullen

This study examines bifacial technology change at the Knob Creek site (12HR484) in Harrison County, Indiana, from the Late Archaic to Early Woodland periods. Through a statistical and attribute analysis of 2,620 lithic flakes it was possible to detect changes in the lithic reduction process over time. The analysis demonstrates that soft-hammer percussion becomes more prevalent during the Early Woodland component of the site. This is a significant change from the hard-hammer percussion industry of the Lower Late Archaic. The Terminal Archaic Riverton component in this study offers one of the few detailed flake-by-flake analyses for this poorly understood lithic …

Over the last 20 years, Gwembe Tonga migrants living in the frontier farming area of Kulaale have witnessed significant declines in non-cultivated “bush” resources due to the conversion of forest and grassland to agricultural uses. “Advise Me!” Understanding Undergraduate Student Perceptions Of Learning In Academic Advising, Schyler Simpson

This dissertation is a theoretically driven empirical analysis of instructional communication in academic advising. It explores the effects of perceived advisor accommodation on advisee learning. Specifically, it examines whether academic advisors employ accommodation communication that influence affect, cognitive learning, and behavioral learning outcomes in advisees. Four hundred and seventeen students were asked to report on their perceptions of learning through an online cross-sectional survey that addressed communication accommodation strategies employed in the advisor/advisee experience. Results show that behavioral learning (measured by intentions) was significantly predicted by advisor inquiry of school-related content (β = .391, p < .01); advisor attentiveness (β = -.169, p < .01); affect toward advising content (β = .154, p < .01); and cognitive knowledge (β = .244, p < .01)].

The object of this review is to summarize the impact of the Internet on the sexual health of adolescents. This article examines the use of websites, blogs and chat rooms as sources for sexual health information for adolescents. The influence of Internet pornography on sexual behaviors and attitudes is addressed. The use of the Internet as a place to find sexual partners is also assessed. During a time of great physical, emotional and sexual change, the Internet is playing a huge role in the decisions adolescents are making, both positive and negative.

This study explores how people with medical concerns seek and perceive social support via Weibo—a social network site in China. The study conducts both a content analysis and an in-depth interview for a comprehensive understanding of the nature of social support on Weibo. Altogether 2548 postings and replies from four Weibo support groups—the Breast Cancer Group, the Arthritis Group, Baby Eczema Group and Children’s Health Group—were categorized into 9 types with a deductive thematic analysis; twenty participants from these four Weibo groups were recruited in the in-depth interview to explore how people seek and perceive social support from Weibo. Microfinance is a development tool used to reduce poverty among extremely poor households. Impoverished households can access lines of credit through microfinance institutions (MFIs), in order to create a new business, smooth household consumption, fund medical emergencies, etc. Many authors postulate that MFIs are drifting from a welfarist to an institutionalist approach to lending.

The local government tax-exempt debt market is a growing, and complex, sector of public finance. As local governments turn to debt financing the factors that contribute to interest costs of that debt have become important considerations for local government officials and politicians. Governance at the local level involves a network of overlapping governments some of which share a tax base. This system of overlapping governments that share a tax base are subject to externalities that arise from taxation, expenditures, and debt.
