Considering A New Paradigm For Alzheimer’S Disease Research, Gillian R. Langley

Using Alzheimer’s disease as a case study, this review argues that it might be time to consider a new paradigm in medical research and drug discovery. The existing framework is overly dependent on often unvalidated animal models, particularly transgenic mice. Translational success remains elusive and costly late-stage drug failure is common. The conventional paradigm tends to overlook species differences and assumes that animal-based findings are generally applicable to humans. Could pathways-based research using advanced human-specific models probed with new tools, including those of systems biology, take centre stage? Other animals, such as mice, rats, rabbits, dogs and monkeys, are widely used as surrogates for humans in fundamental medical research. This involves creating disorders in animals by chemical, surgical or genetic means, with the aim of mimicking selected aspects of human illnesses. It is a truism that any model or surrogate is not identical to the target being modelled. So, in medical research, experiments using animals or cell cultures or even healthy volunteers instead of patients (being the target population with the target illness) will inevitably have limitations, although these will be greater or lesser depending on the model.

Medicaid Expansions & Public Health Spending: Cross-Subsidies, Complementarities, And Crowd-Out, Glen P. Mays

In this paper we estimate the causal impact of state Medicaid enrollment expansions and expenditures on state and local resources allocated to other public health programs and services. Using a quasi-experimental design with instrumental variables estimation, we find evidence that increased Medicaid spending leads to reduced governmental spending on other public health services, consistent with a crowd-out effect. Over 10 years, such crowd-out has the potential to diminish the health status improvements generated through health insurance coverage expansions.

Correlates Of Social Anxiety, Religion, And Facebook, Lee Farquhar, Theresa Davidson

This study examined how religiosity, network homophily, and self-monitoring relate to social and Facebook-specific anxiety, role conflict, and Facebook Intensity. Correlation analyses indicate a connection between Facebook use and anxiety, as well as a link between religiosity and anxiety. We found that Role Conflict correlates with Facebook Intensity, Facebook specific Anxiety, and Social Anxiety. Regarding religiosity, those who prefer aliteral interpretation of the Bible, attend church more frequently, and pray more often have higher anxiety. Facebookers who are higher self-monitors have a less homophilous Facebook network and are less likely to identifytheir religious views on Facebook.

An Innovative Approach To Improving Father-Child Relationships For Fathers Who Are Noncompliant With Child Support Payments: A Mixed Methods Evaluation, John R. Gallagher, Joan R. Rycraft, Tommy Jordan

Journal of Adolescent and Family Health

Mixed methods were used to evaluate the effectiveness of the Fathers Offering Children Unfailing Support (FOCUS) program. FOCUS is a diversion program which is designed to offer an alternative to incarceration for fathers who are noncompliant with child support payments. Quantitative data were collected through a pretest/posttest design (n = 55) and qualitative data were collected through telephone interviews with FOCUS instructors (n = 2) and community key stakeholders (n = 5) and focus groups with FOCUS participants (n = 76). Kentucky's implementation of the Affordable Care Act has included early successes with insurance coverage expansion through Medicaid and a state-operated health insurance exchange. Signals of improvements in health care accessibility and delivery of preventive services are evident in the first year after coverage expansions. Challenges associated with political opposition, delivery system transformation, and public health financing remain on the state's policy agenda.

OBJECTIVES: A growing body of evidence indicates that the delivery of public health activities varies widely across states and communities, creating missed opportunities for prevention as well as inequities in health protection. Measures of quality in public health are needed to guide public health improvement initiatives and to support research on the comparative effectiveness of alternative public health strategies.
