Problem: The subject organization (SO) is a Federally Qualified Health Center (FQHC) with an internally developed incident reporting system. The SO wanted to improve patient and employee safety using data from incident reports, but the incident reporting system did not give enough information to recognize patterns and develop countermeasures.

Context: Supervisors welcomed the opportunity to learn more about incident report follow-up and conducting root cause analysis (RCA). New York City's Rikers Island has a medically assisted treatment (MAT) program for detainees who are addicted to opioids, providing buprenorphine or methadone. For many locked up there, though, Rikers is only a way station before a trip upstate to prison. Even now, over 30 years after its treatment program began, only six other correctional facilities in New York offer pilot opioid treatment programs, which are available only to limited segments of their respective populations.

Alfonso Xicali López saw his mother for the first time in almost two decades. He is participating in a program called Raíces de Puebla, which means Roots of Puebla in Spanish. The program pays for tourist visas and plane tickets for the relatives of undocumented Mexican immigrants who haven’t seen their family members in a decade or more. The government of Alfonso’s home state, Puebla, funds the program. They aim to maintain family connections and reward immigrants who are active members of their communities.

For the last 25 years, evangelical leaders have been shipping off opioid addicts in Puerto Rico to cities in the mainland US, mainly New York City, Chicago and Philadelphia--under the promise they’ll receive state-of-the-art rehabilitation treatment, only to end up in unregulated transitional homes and flophouses where they don’t receive proper medical care or psychotherapy. In turn, many of these unregulated transitional homes, also run by evangelical leaders, may charge Medicaid kickbacks from their “patients.”

In this autoethnography, I am concerned with cultural relevance as an experience of a scholar-practitioner educational leader. I question my own cultural competence as a teacher and school principal. Turning a reflective gaze on my lived experiences as an educator creates a space in which I attempt to make meaning of the phenomenon of culturally relevant practices in the field of education. As an act of pedagogical and personal meaning-making, this autoethnographic work centers on the value of cultural relevance as informed by scholarly practice.

Anxiety disorders as a stand-alone diagnosis are not recognized as a treatable mental health issue within the social services mental health realm. There are mixed results regarding if anxiety disorders are linked to suicide ideation and suicide attempts. The aim of this systematic review was to discover if there is a link between anxiety disorders and suicide ideation and suicide attempts within the current literature. New York City’s public housing, known as NYCHA, is a popular choice for disabled and elderly adults. 40% of NYCHA residents receive support from government programs intended to help older New Yorkers and New Yorkers with disabilities, and 32% of NYCHA households include an elderly person. Contrast that to New York City as a whole, where 11.2% of the population is disabled and 13% is elderly. In 2015, a riot broke out at a federal prison in a small Texas town not far from the U.S.-Mexico border. Prisoners tore through the kevlar tents of their dorms and a fire broke out, rendering the private prison uninhabitable. The facility was an economic engine for the impoverished, Rio Grande Valley county, so when the facility closed, it left the county $2 million short of its estimated budget, with $78 million in unpaid bonds and 400 fewer jobs for a population of about 21,000 people.
