In many applications, the dataset for classification may be highly imbalanced where most of the instances in the training set may belong to some of the classes (majority classes), while only a few instances are from the other classes (minority classes). Conventional classifiers will strongly favor the majority class and ignore the minority instances. The imbalance problem can occur in both binary data classification and also in ordinal regression. Ordinal regression is a supervised approach for learning the ordinal relationship between classes. Background: Though lead contaminated waste sites have been widely researched in many high-income countries, their prevalence and associated health outcomes have not been well documented in low- and middle-income countries.

Methods: Using the well-established health metric disability-adjusted life year (DALY) and an exposure assessment method developed by Chatham-Stephens et al., we estimated the burden of disease resulting from exposure to lead at toxic waste sites in three Latin American countries in 2012: Argentina, Mexico and Uruguay. Background

Buprenorphine is a semi-synthetic opioid used for the treatment of opioid dependence. Opioid use, including buprenorphine, has been increasing in recent years, in the general population and in pregnant women. Consequently, there has been a rise in frequency of neonatal abstinence syndrome (NAS), associated with buprenorphine use during pregnancy. The purpose of this study was to investigate correlations between buprenorphine and buprenorphine-metabolite concentrations in cord blood and onset of NAS in buprenorphine exposed newborns.

Downstream regulatory element antagonist modulator (DREAM) is involved in various interactions with targets both inside and outside of the nucleus. In the cytoplasm, DREAM interacts with the C-terminal fragments of presenilins to facilitate the production of β-amyloid plaques in Alzheimer’s disease. In the nucleus, Ca²⁺ free DREAM directly binds to specific downstream regulatory elements of prodynorphin/c-fos gene to repress the gene transcription in pain modulation. Background Modified Vaccinia Ankara MVA-BN^® is a live, highly attenuated, viral vaccine under advanced development as a non-replicating smallpox vaccine. In this Phase II trial, the safety and immunogenicity of Modified Vaccinia Ankara MVA-BN^® (MVA) was assessed in a 56–80 years old population.

Methods MVA with a virus titer of 1 x 10⁸ TCID₅₀/dose was administered via subcutaneous injection to 56–80 year old vaccinia-experienced subjects (N = 120). Aim

Analysis of wastewater samples can be used to assess population drug use, but reporting and statistical issues have limited the utility of the approach for epidemiology due to analytical results that are below the limit of quantification or detection. Unobserved or non-quantifiable—censored—data are common and likely to persist as the methodology is applied to more municipalities and a broader array of substances. We demonstrate the use of censored data techniques and account for measurement errors to explore distributions and annual estimates of the daily mean level of drugs excreted per capita.

A lack of surveillance systems is an impediment to public health intervention for perennial vector-borne disease transmission in northern tropical savanna region of Kenya. The population in this area are mostly poor nomadic pastoralists with little acquired functional immunity to Plasmodium falciparum, due to infrequent challenges with the parasite. A common characteristic in tropical savanna climatic zone is the availability of riverbeds that have anthropogenic boreholes that provide malaria vector mosquitoes, such as Anopheles gambiae s.l and Anopheles funestus, with aquatic refuge habitats for proliferation and endemic transmission to proximity human households during the dry-season. Total Versus Partial Splenectomy In Pediatric Hereditary Spherocytosis: A Systematic Review And Meta-Analysis, Leonardo Guizzetti

To compare the clinical effectiveness of total (TS) or partial (PS) splenectomy in pediatric hereditary spherocytosis, a systematic review and meta-analysis was performed (PROSPERO registration CRD42015030056). There were 14 observational studies comparing pre- and post- operative hematologic parameters. Secondary outcomes include in-hospital infections, surgical complications, symptomatic recurrence and biliary disease. TS is more effective than PS to increase hemoglobin (3.6 g/dL vs 2.2 g/dL) and reduce reticulocytes (12.5% vs 6.5%) after one year; outcomes following PS are stable for at least 6 years. There were no cases of overwhelming post-splenectomy sepsis. A population-based patient registry is needed for long-term follow-up.

BACKGROUND: A homologue of the ecdysone receptor has previously been identified in human filarial parasites. As the ecdysone receptor is not found in vertebrates, it and the regulatory pathways it controls represent attractive potential chemotherapeutic targets.

METHODOLOGY/ PRINCIPAL FINDINGS: Administration of 20-hydroxyecdysone to gerbils infected with B. malayi infective larvae disrupted their development to adult stage parasites. A stable mammalian cell line was created incorporating the B. malayi ecdysone receptor ligand-binding domain, its heterodimer partner and a secreted luciferase reporter in HEK293 cells. This was employed to screen a series of ecdysone agonist, identifying seven agonists active at sub-micromolar concentrations. …

This legislation permits casinos and slot parlors in Massachusetts under the regulatory auspices of the Massachusetts Gaming Commission (MGC). Research is required to identify factors contributing to decisions to enter and/or breach self-exclusion orders, or to self-excluded gamblers to seek additional treatment. Clinical characteristics of self-excluders remain relatively unknown in addition to factors that differentiate gamblers who do or do not breach. Limitations of current programs are that gamblers can self-exclude from only one or a limited number of venues. We develop surveys of gambling problems in the general Danish population. We compare several popular survey instruments of gambling behavior and gambling propensity to assess if they differ in their classification of individuals. We also examine correlations with standard survey instruments for alcohol use, anxiety, depression and impulsiveness. A feature of our design is that nobody was excluded on the basis of their response to a “trigger,” “gateway” or “diagnostic item” question about previous gambling history, allowing us to check for sample selection bias. A recent systematic review of population studies conducted over the last 12 years indicated that 57.5% of individuals meeting criteria for a gambling disorder have a comorbid substance use disorder, 60.1% a nicotine dependence, 57.5% a mood disorder, and 37.4%, any anxiety disorder. In a proportion of these, the comorbid condition predates the onset of gambling disorders suggesting these might represent a vulnerability factor. These findings suggest important implications for gambling prevention and treatments. Characterization of surface gas permeability measurements on a variety of natural and engineered building materials using two relatively new, non-destructive surface permeameters is presented. Surface gas permeability measurements were consistent for both laboratory and field applications and correlated well with bulk gas permeability measurements. This research indicates that surface permeability measurements could provide reliable estimates of bulk gas permeability; and due to the non-destructive nature and relative sampling ease of both surface gas permeability tools, it is possible to quantify the range of the spatial autocorrelation, heterogeneity, and anisotropy in porous building materials and their degree of degradation from weathering.

Objective: Because partners are an important and unpaid resource in cancer care, understanding how destructive, controlling or interfering partner behaviors influence women's cancer care may be particularly relevant for health care providers seeking to provide cancer care and enhance recovery. Using a new measure of partner interfering behaviors in cancer care (PIB-C), we investigated whether women with a recent cancer diagnosis who additionally endorsed any PIB-C would report (a) more symptoms of depression and stress, and (b) lower Functional Assessment of Cancer Therapy (FACT-G) and lower Functional Assessment of Chronic Illness Therapy-Spiritual Well-being (FACIT-SP) scale scores indicating poorer quality of …
