In an attempt to prevent acute low back pain from becoming a chronic disability problem, an earlier study developed a statistical algorithm which accurately identified those acute low back pain patients who were at high risk for developing such chronicity. The major goal of the present study was to evaluate the clinical effectiveness of employing an early intervention program with these high-risk patients in order to prevent the development of chronic disability at a one-year follow-up. Approximately 700 acute low back pain patients were screened for their high-risk versus low-risk status. On the basis of this screening, high-risk patients were …

Almost every leading work on the law of confidence mentions the possibility ofa declaration of a constructive trust as a remedy for a claim involving an abuseof confidence. Apart from the Canadian Supreme Court, no other appellatecourt in the Commonwealth has seriously debated this issue. This paperinvestigates the legitimacy of the use of the constructive trust in this context.

This paper develops a model which is able to forecast exchange rate turmoil. Our starting point relies on the empirical evidence that exchange rate volatility is not constant. In fact, the modeling strategy adopted refers to the vast literature of the GARCH class of models, where the variance process is explicitly modeled. Further empirical evidence shows that it is possible to distinguish between two different regimes: îordinaryî versus îturbulenceî. Low exchange rate changes are associated with low volatility (ordinary regime) and high exchange rate devaluations go together with high volatility. This calls for a regime switching approach. Market Structure And Performance: An Anti-Trust Story Of Endogenous Growth, Hing-Man Leung

Since Schumpeter, a major concern has been: what monopoly does to growth? Monopoly’s static, allocative inefficiency is well established. How much this is offset by its dynamic progressiveness is unclear. First, using the empirical literature, we argue that the presumed progressiveness of monopoly must be rejected. Second, we extend the endogenous growth model to obtain a full Pareto ranking of competition, monopoly, Cournot and Bertrand. Competition beats Cournot, which in turn beats monopoly. Growth rate is invariant with structures, which accords well with empirical evidence. Bertrand happens to share the ranking with competition. The findings have a strong anti-trust overtone.

In 1991-92, the Research and Training Center on Rural Rehabilitation (RTC: Rural) evaluated 45 state Vocational Rehabilitation (VR) agencies’ policies and procedures regarding self-employment for people with disabilities. Based on this analysis, researchers developed and broadly disseminated a model VR self-employment policy (Arnold & Seekins, 1994) and were successful in their efforts to have self-employment included as an employment outcome in the 1998 Reauthorization of the Rehabilitation Act. In 2002, RTC: Rural investigated state policies again to see how they had changed and what trends had developed over the intervening decade. Outcome following hypoxic-ischemic encephalopathy (HIE), a neurodegenerative process caused by prolonged asphyxiation during birth, can vary between minimal impairment and cerebral palsy, mental retardation, or death (30%) (McCulloch, Taylor, & Whyte, 1991). Prognosis after HIE is often difficult to establish because traditional predictors, such as Sarnat scores, Apgar scores, and pH, do not always account for adequate variance in outcome. The prognostic utility of MR Spectroscopy in the pathogenesis of asphyxia is promising (Wyatt, 1994). Innovative research indicates that MR Spectroscopy can accurately predict outcome at one year in 91% of neonates with central nervous system injuries (Holshouser et al., …

OBJECTIVE:
To examine the impact of introduction of an alcohol-based hand rub on hand hygiene knowledge and compliance and hand colonization of healthcare workers (HCWs) in a long-term-care facility (LTCF).
METHODS:
Two floors of an LTCF participated. Ward A used the hand rub as an adjunct to soap and water; ward B was the control. HCWs' hands were cultured using the bag-broth technique for Staphylococcus aureus, gram-negative bacilli (GNB), Candida, and vancomycin-resistant enterococci (VRE). HCWs completed a questionnaire at baseline and after an educational intervention and introduction of rub.
RESULTS:
Hand hygiene practices, knowledge, and opinions did not change after …

Political leaders who establish policies and priorities for the nation have influence on how the FSM's sole postsecondary education institution, the College of Micronesia, Federated States of Micronesia (COM-FSM) sets its goals and priorities. Nearly 14 million Americans have serious problems because of their drinking including eight million men and women who suffer from alcoholism, a chronic disease. Alcoholism can be clinically diagnosed and people with the disease typically experience many additional alcohol-related medical complications. Research demonstrates that treatment can save lives, restore families, reduce health care costs, increase productivity in the workplace and make our communities safer.

The leper colony on the Hawaiian island of Molokai has been covered in a number of books over the years, both fiction and nonfiction, many focusing on the efforts of Father Damien of Belgium, who sacrificed his own health to improve the conditions of the exiles. This latest volume is written from the perspective of a 12-year-old Hawaiian girl, Liliha, who contracts the disease from her cruel grandmother, and the novel is set in the years before Father Damien arrived at the colony in 1873. The paper presents an account of the scholarly work of Canadian sociologist, feminist, theorist and activist, Dorothy E. Smith, leading up to her development of institutional ethnography as "a sociology for people." Drawing on selected writings, the author discusses some of the major ideas, debates and practical influences that are part of Smith's scholarly trajectory. The line of thinking that is illustrated is how her feminism was integral to her celebrated critique and re-writing of sociological method.

In this essay, I reflect on the work of Lorraine Code on Rhetorical Spaces and the work of Dorothy Smith on Institutional Ethnography to explore how stories are translated and seen as though looking through the different turns of a kaleidoscope. The stories I am referring to here are intake stories in human service agencies. The question is how do the front line human service workers translate the noise of everyday/night life of the "client" into the human service jargon/forms. This article examines how Asian immigrant women manage the demands of family,job training, and paid work in their new society. Using institutional ethnography, a feminist research strategy developed by Dorothy Smith, the study begins with the women's experiences to explore the extended social relations which give shape to them. The study argues that among those extended relations are the organization of the labor market in the contemporary period, immigration legislation, and the ideological practices embedded in developing, managing, and administering public policies such as job training. A Child's Death: Lessons From Health Care Providers' Texts, Nancy M. Bell, Marie L. Campbell

This article originates from a research study that explores 'what happened' to a 10-year-old child with Rett syndrome, who died from "severe malnutrition" according to a Coroners Service inquest jury. The inquest evidence analyzed, using institutional ethnography, shows that approximately one week prior to this child's death three health care providers (an emergency physician, a hospice volunteer and a home care nurse) conducted individual assessments of the child. Child protection workers were also involved. Textual analysis of the health care providers' records shows how the child was officially and textually constructed as 'dying from a terminal illness' in contrast to …

The topic of this chapter is the social psychology of cross-cultural interaction. We discuss the psychological processes that take place during and after meetings between individuals and groups who differ in their cultural backgrounds. We identify two types of cross-cultural contact: a) meetings that occur between two societies when individuals travel from their place of origin to another country for a specific purpose and a limited amount of time, such people being called sojourners in the literature; and b) meetings within multi-cultural societies among its ethnically diverse permanent residents. The United States Air Force (AF) has named Information Superiority the core competency "upon which all the other core competencies rely". In order to achieve Information Superiority, deceptive communication must be minimized. According to researchers, deception occurs when communicators control the information contained in their messages to convey a meaning that departs from the truth. This research draws on Biros, George, and Zmuds' (2002) deception research model to determine if training to detect deception will improve a person's deception detection performance in a computer-mediated environment. The United States Air Force is in the process of implementing GeoBase, an initiative to change how geospatial information resources are being acquired, implemented, exploited, and sustained on USAF installations around the world. Using GeoBase increases situational awareness and minimizes decision risk for all installation personnel GeoBase is a geographic information system, using commercial-off-the-shelf software to provide one source for spatial information on an installation. GeoBase uses imagery as one layer of information. The accuracy of the imagery directly impacts the effectiveness of GeoBase.
