A Research Agenda For Economic Evaluation Of Substance Abuse Services, Michael T. French, M. C. Drummond

Michael T. French

Economic analyses of substance abuse interventions play a critical role in informing the decision makers involved in funding these programs. Despite the emergence of new and more effective interventions, the adoption of costlier services still demands justification based on economic evidence. Updated and more rigorous economic information allows patients, health care professionals, insurance companies, policymakers, and others to allocate scarce resources more efficiently. To prepare for the next wave of addiction health services research, this article presents background information on the economics of addiction health services, reviews recent empirical and methodological contributions, and provides 15 research recommendations.

Objective
This study reports the prevalence and comorbidity of depression in two large samples of black and white young adult women.
Method
Clinical interviews of participants in a follow-up study of the National Heart, Lung, and Blood Institute Growth and Health Study (NGHS-Wave II; N = 378) were contrasted with a subsample of the National Comorbidity Survey (NCS; N = 3749) to examine the rates and comorbidity of lifetime major depressive disorder in black and white women using methodology described by Kraemer (1995). The prevalence of depressive symptoms from adolescence through young adulthood was examined in 1,146 African-American adolescent girls and 1,075 Caucasian adolescent girls who completed the Center for Epidemiological Studies of Depression scale. Caucasian girls’ scores decreased over time, whereas scores for African-American girls were fairly consistent. Future studies are needed to examine age-specific risk factors in adolescent girls.

Objective: Two studies explored in nonclinical samples variables that may influence meal size assessments.
Research Methods and Procedures: In Study 1, the contribution of dietary restraint, weight, depressive symptoms, and gender to ratings on the Food Amount Rating Scale was examined in 153 white women and 121 white men; in Study 2, the contribution of dietary restraint, weight, ethnicity, and ethnic group identification to Food Amount Rating Scale ratings was examined in 67 Asian, 81 Hispanic, and 107 white women.

Background. To examine whether adolescent depressive symptoms predict young adult body mass index (BMI) and obesity in black and white women.
Method. Participants included 1554 black and white adolescent girls from the National Heart, Lung, and Blood Institute Growth and Health Study (NGHS) who completed the Center for Epidemiological Studies – Depression Scale (CES-D) at ages 16 and 18 years.
Results. Regression analyses showed that depressive symptoms at both ages 16 and 18 were associated with increased risk of obesity (BMI[gt-or-equal, slanted]30) and elevated BMI in young adulthood (age 21) in both black and white girls. Black girls exhibited a …

We develop a microstructure model that, in contrast to previous models, allows one to estimate the frequency and quality of private information. In addition, the model produces stationary asset price and trading volume series. We find evidence that information arrives frequently within a day and that this information is of high quality. The frequent arrival of information, while in contrast to previous microstructure model estimates, accords with nonmodel-based estimates and the related literature testing the mixture-of-distributions hypothesis. To determine if the estimates are correctly reflecting the arrival of latent information, we estimate the parameters over half-hour intervals within the day. …

Background. There is an ongoing debate about the best way to classify eating disorders. This study examined potential subtypes of bulimia nervosa. Method. Latent class analysis (LCA) was used to identify subtypes of bulimic symptomatology, utilizing data from 234 respondents in a cohort of black and white young women (n=2054). Participants were administered gated screening questions from the Eating Disorders Examination to determine DSM-IV diagnoses of eating disorders. Results. A 3-class solution was judged best. Class 1, the ‘purger subtype’ (n=116), was characterized by vomiting, the use of fasting/diet pills, and relatively little bingeing. Class 2, the ‘binger subtype’ (n=97) …

Objective The current study examined the prevalence and clinical significance of night eating syndrome (NES) in a community cohort of Black and White women. Method We assessed 682 Black and 659 White women for NES, eating disorders, and psychiatric symptomatology. Results The prevalence was 1.6% (22 of 1,341; Blacks [n = 20]; Whites [n = 2]). Comparisons between identified Black women and the remaining Black participants revealed no significant differences in obesity, psychiatric comorbidity, or self-reported psychiatric distress. Objective The current study examined health services use during the past 12 months in a sample of young women with a history of an adolescent eating disorder (bulimia nervosa [BN] or binge eating disorder [BED]). Method A community sample of 1,582 young women (mean age = 21.5 years) was classified, based on a screening interview (and, for eating disorder diagnosis, confirmatory diagnostic interview), into one of three groups: BN or BED (n = 67), other psychiatric disorder (n = 443), and no adolescent psychiatric disorder (n = 1,072). Objective: Problem alcohol use among elderly persons can have a variety of health-related consequences, complicating management of chronic illnesses and increasing health care utilization and costs. This study evaluates the economic cost and benefits of brief intervention for at-risk drinking older adults. Method: A controlled clinical trial with 24-month follow-up tested effectiveness of brief physician advice in reducing alcohol use, health care utilization and other consequences among older (age 65 or older) adult problem drinkers. Of 6,073 patients screened for problem drinking in 24 community-based primary care practices in Wisconsin, 158 patients met inclusion criteria and were randomized into control …

Doing research in dangerous and post-conflict areas require specific methodologies and techniques. This article explores the cross-cultural collective interviews and focus groups methods developed and adapted during the a fieldwork among Maya indigenous communities in Chiapas, Mexico. The author reviews the challenges and advantages of adopting such culturally intelligent methods focusing on the promotion of justice and reconciliation among indigenous communities in conflict.

Two studies investigated whether participants' motivational state and the context in which attitude reports are made influence food attitudes. Specifically, these studies examined whether hunger and the time-typicality of foods (i.e. match or mismatch between the time when a food is typically eaten and the time the attitude is reported) interact to influence reported attitudes. We test traditional assumptions about the volatility of mass opinion in times of national crises using data about views of terrorism from national surveys of the United States general public in 1995 and 1997, findings from a national survey immediately following the terrorist attacks of September 11, 2001 (9/11), and panel data from a follow-up survey in 2002. We compare public assessments of the threat of terrorism, willingness to restrict speech to prevent terrorism, support for employing conventional military force against countries that support terrorism, and levels of certainty about culpability required prior to using military force. Objective

To incorporate selected items from several existing instruments to create a comprehensive multi-factorial instrument to measure problematic eating behaviors in young children and to examine the prevalences and correlates of these behaviors.

Method

A community sample of young mothers (n = 93) completed the inventory of problematic eating behaviors for their 36-month old children.

Results

A clear understanding of help-seeking intentions and behavior is fundamental to the identification of factors that can be modified to facilitate young people’s help-seeking when they are psychologically distressed or suicidal. Despite considerable research on help-seeking intentions and behavior for personal-emotional or distressing problems, integrating prior research has been impeded by a lack of consistent and psychometrically sound help-seeking measures. The General Help-Seeking Questionnaire (GHSQ) was developed to formally assess two aspects of help-seeking: (1) current intentions to seek help from different sources for different problems; and (2) quantity and quality of previous professional psychological helping episodes.
